Social Recreation Account Registration

Welcome to our Social Recreation Program! This initial account registration process ensures that we can provide the best experience tailored to your interests and needs. The account setup includes gathering information about your background, preferences, and any specific accommodations required. Here's what to expect:

1. Personal Information:

  • Name, age, and contact details
  • Emergency contact information
  • Relevant health or medical information (allergies, conditions, etc.)
  • Regional Center POS / Authorization information
  • IPP Documents

2. Accommodations:

  • Any specific needs or preferences (e.g., physical accommodations, sensory considerations, or preferred communication methods)

3. Consent and Agreements:

  • Consent for participation, media release (if applicable), and liability waiver
  • Code of conduct agreement to foster a positive, inclusive environment
